SeniorLink
provides the link between you, your loved ones, and the services
they need. SeniorLink is a non-sectarian service
and available to people of all religions.
"I'm
86 years old and it's getting more and more difficult to manage
things on my own. I need a little help but I want to maintain
as much independence as possible."
"My
mom lives alone in an apartment and is having difficulty taking
care of herself. I feel helpless living so far away."
SeniorLink
provides the tools you need to make the best choices for yourself
or those you care about. Our care management team of experienced,
compassionate professionals are here to help you make difficult
decisions with more confidence and ease.
Our goal is
to help seniors to live safely and independently with dignity
and contentment. When you contact SeniorLink,
a geriatric care manager will:
- Gather
information to determine whether SeniorLink
is for you.
- Prepare
an individualized care plan with recommendations for future
action.
- Review
the plan with you.
If you and
your family would like us to implement the plan, we utilize in-house
and outside support services.
We visit
you or your loved one on a regular basis and and convey an updated
report to the appropriate person after each visit.
SeniorLink
offers peace of mind to seniors and their families. If you have
concerns about senior care, we are here to help.

Caring
for Our Community: JFS-LV's Senior Services
They come from
all over our community, and each has his or her own story.
Mrs. D. can't
believe her eyes. Where her friendly pharmacist used to hand out
prescriptions and advice--right there behind the counter--a stranger
stands, selling office equipment. And the corner luncheonette
is gone. Even her old neighbors have moved away. This was not
what 72-year-old Mrs. D. expected when she returned to the Lehigh
Valley after many years in another state. Feeling lost, confused
and alone, Mrs. D. turns to JFS-LV. Through the agency's Counseling
Program, she finds emotional support and practical solutions to
help her adjust to her old/new environment.
Mrs. W.'s
only child, a son, has moved to Pittsburgh to take a new job.
But Mrs.W. is not alone; before he left, her son arranged for
JFS-LV's Care Management Program to be his "eyes and ears."
Mrs. W.'s geriatric care manager visited Mrs. W. at her apartment
to determine exactly what she needs to continue living safely
and independently. He helped secure and coordinate homemaking
and meal-delivery service for her. He continues to visit her regularly,
helping her keep track of her doctor appointments and monitoring
her overall well-being.
Mr.N. is homebound
and in precarious health. A very limited income makes life even
harder. Fortunately, Mr. N. has assistance from JFS-LV. His agency
social worker brings Mr. N. meals from the agency's Kosher Food
Pantry. He also provides counseling for the seventy-nine-year-old
man in his own home.
Mrs. O. puts
on her favorite earrings and joins her friends in the recreation
room. It is the time she most looks forward to˜when JFS-LV's
senior services coordinator brings Programs of Jewish Interest
to the assisted living community where she lives. Whether it is
reminiscing about childhood Chanukahs or learning new facets of
Jewish art, Mrs. O. says that "Jewish Family Service brings
us the Yiddishkeit that is missing here."
Mr. C., age
82, is worried: Problems with his eyesight have forced him to
give up driving. How will he get his groceries, get to his doctor
appointments?
Around the
corner from Mr. C., 65-year-old Mrs. L. wakes up on her first
morning of retirement. She looks forward to things she never had
time for before. Mostly, she's always wanted to help people as
a volunteer. As luck will have it, Mr. C. and Mrs. L. both call
JFS-LV. Before long, they meet each other through JFS-LV's Volunteer
Program, when Mrs. L. becomes Mr. C.'s volunteer driver.
Retirees Mr.
and Mrs. K. lead active lives and like to keep pace with the latest
developments. At JFS-LV's Over-50 Brunch and Learn Series, they
learn about health, legal and other topics that expand their knowledge
and further improve their quality of life.
Dr. J. is
looking for senior housing for her parents. She calls JFS-LV's
Information and Referral Service. An agency social worker provides
valuable information about senior apartments and assisted living
communities.
Mr. M. has
lost his beloved wife of forty-seven years. JFS-LV's Bereavement
Support Group connects him to others who are also coping with
grief, and he begins to find solace and peace.
As shown by
the stories above, seniors and their families come to JFS-LV with
many different needs. JFS-LV meets these varied needs with services
that are personally tailored to them. For more information on
how JFS-LV can help you or your loved ones, call 610-821-8722. |
