All requests for service must be accompanied by a completed JFS-LV Service Request form for each person who is requesting service. You may either complete the two page form at the JFS-LV office or download both pages as PDF (Acrobat Reader) files, complete them, then either fax (610-821-8925) or bring them to the JFS-LV office.
